This ampoule in the syringe contains a drug to relieve pain after major surgery. The drug is given into the epidural space using a special needle.
The needle hits the bullseye in the same place as the scalpel to symbolise relieving the pain. Putting the ampoule in the syringe needed a bit of complex work in Photoshop since in real life the ampoule is too big.
